Important Travel Update
Ebola Declaration Form
To help prevent the spread of Ebola, the Centers for Disease Control and Prevention (CDC) requires all international travellers flying to the United States from 3 November 2026 to complete a CDC Ebola Attestation Form before departure. As part of the declaration, passengers must confirm that they have not visited the Democratic Republic of the Congo (DRC) within the previous 21 days.
The form can be completed electronically via your airline's app before travel, or the form will be provided by the airline at check-in. Please note that this requirement applies to all travellers flying to the USA, including those who are transiting through the United States and connecting to another destination. Further information on the new requirement can be found on Order: Requirement for Attestation by All Persons Traveling by Air to the United States | Port Health | CDC
